HTML or print output?

Are there some big tables in there? Table processing is a bit
slow because there's so much bookkeeping to be done to make
sure that the calculated column number is correct.

For HTML, are there a lot of RefEntrys in there?  The code that
calculates previous and next entries is a bit sub-optimal.  It's
on my list...
