Ah, thanks. With your hint about tex, I seem to have found the answer at http://cyberelk.net/tim/docbook/selfdocbook/x173.html! On an unrelated issue, when I enable two-side in my DSSSL stylesheet, my page numbering in the table of contents stops working.
