It's for a book, but we haven't updated to BITS yet (still using NLM 3.0),
another reason to that.

If (and only if) you have the flexibility to enhance JATS for your own use you
might consider adding the <index> structures from BITS to your use of JATS.

sample of a nested index.

This is only appropriate if you have no requirement that your JATS documents
be processed by someone using JATS as published.
