Thanks Debbie.
This is useful. I think this will do it. However I do think there is still a
need for an inline-media element that caters for this situation and identifies
"integral" media as an inline call.  I'll make a request.

Ah, I get it. You want <media> to act as a highlighting element,
surrounding existing text to hold the link to the audio.
Sorry, neither <graphic> nor <media> work in this fashion.

However, <named-content> may work. There are at least two
possibilities:

 - <named content> can contain an <ext-link>

 - <named-content> takes the following attributes, some of which
    are linking:

specific-use Specific Use

Without knowing more about what you want to do, it sounds to me like you are
looking for a <fig> containing a <media>.
