That's not the right URI. And it's not wellformed. But that's
probably just a typo.

| <xsl:include href="header.xsl">
| </xsl:include>

Try

  <xsl:include href="header.xsl"/>

It's possible that the processor is complaining because xsl:include
isn't empty in your document (that newline counts as data).
