Yes,  with Xalan there is an extension called redirect that will do just
that.  The stylesheet below shows how it is done. I modified
your source data just a bit.  It is included after the stylesheet.

Stylesheet:

<xsl:template match="/doc/image">
<redirect:write file="{@name}.{@extension}">
     <xsl:value-of select="."/>
</redirect:write>
