The tool you want is called HTML Tidy: http://www.w3.org/People/Raggett/tidy/
This tool cleans up HTML, and can also convert it to XML or XHTML. I've used it to pre-process HTML documents before sending them to an XSL processor. It works great, just be sure to configure it the right way.
As far as I know you can't do it with xslt which is meant to take xml as input and produce xml or text or html as output, your html file is not well formed xml (the INPUT tags are not closed) all xsl processor should reject them.
