See section 7.5 of the XSLT spec.

There's a simpler way:

<xsl:template match="/">
  <xsl:copy-of select="."/>
</xsl:template>

But you can't modify that to make any changes.

Turn to disable-output-escaping as a last resort only. If you're using it as
a beginner then it means you haven't understood the basic principle of the
language, which is that you write nodes to the result tree, not tags to the
output file.
