Emerick,

The deafening quiet you hear may be because:
1. You're really asking an HTML question, not an XSL question
2. HTML is really lousy for printing

XSLFO is not absolutely required for this, but it's probably the best way to go for a "lights out" process.

If it's a one-off, I would generate really clean HTML, import it to a word processor or DTP app, and add the headers there.

Does anyone have an HTML -> FO stylesheet that someone like Emerick could use at least to mock up an FO target? That would be a useful thing, I think. (The stylesheet that converts a stylesheet targetting HTML, to one targetting FO, I'll ask about another day.)
