Your question was anwered earlier, but you should be aware:


           <fo:root xmlns:fo = "http://www.w3.org/1999/XSL/Format/1.0";>
...
                   <fo:sequence-specification>
                       <fo:sequence-specifier-single
                           page-master-name="only"/>
                   </fo:sequence-specification>

Your use of this namespace URI and some sample constructs are not in the final version of XSLFO.


You should review http://www.w3.org/TR/xsl/ for the latest.
