[xsl] node vrs node//node?

Given this xsl:

<xsl:template match="form//checkbox[not(@checked) or @checked != 'yes']">
 <xsl:apply-templates/>
 <input type="checkbox" name="{@name}" value="{@value}"/>
</xsl:template>

<xsl:template match="form//checkbox">
 <xsl:apply-templates/>
 <input type="checkbox" name="{@name}" value="{@value}" checked="yes"/>
</xsl:template>


And this xml:

<checkbox name="i2a" value="1" checked="yes">One</checkbox>
<checkbox name="i2b" value="2">Two</checkbox>
<checkbox name="i2c" value="3">Three</checkbox>


I get this output:

One<input type="checkbox" name="i2a" value="1" checked>
Two<input type="checkbox" name="i2b" value="2" checked>
Three<input type="checkbox" name="i2c" value="3" checked>

Not this:

One<input type="checkbox" name="i2a" value="1" checked>
Two<input type="checkbox" name="i2b" value="2">
Three<input type="checkbox" name="i2c" value="3">


Cany anyone tell me what I'd doing wrong? If I leave the check to make
sure the checkboxes are in a form out it seems to work fine. Obviously I
mean only for the 'checked' box to be checked by default. Any help is
appreciated. Thanks.
