You need to do:

<xsl:template match="*" mode="escape-apos">
  .. the identity template ..
</xsl:template>

<xsl:template match="text()" mode="escape-apos" name="escape-apos">
  .. your recursive template ..
</xsl:template>

<xsl:apply-templates select="xx:node-set($RTF)" mode="escape-apos"/>
