Unfortuantely disable-output-escaping don't seem to be working at all for some reason. As I mentioned, it has no effect on attribute values. There is no way in standard XSLT to get an unquoted ampersand in an attribute value, short of using the text output method and and handling all issues of XML linearisation within the stylesheet.
