Much of your problem may be in trying to handle arbitrary TEI. The DTD is loose enough, as you are finding, to make for quite a challenge in the general case. :->

Most (or let's just say all) TEI will not use the full tag set, and it'll be considerably easier to write stylesheets to handle the TEI subset used in a particular project. If there's any way you can do that, consider it.

You might also find splitting your transform into two passes helpful. First pass to normalize (e.g. makes all numbered divs into plain divs; XSLT doesn't need the numbering!), and a second pass to provide the formatting. The formatting stylesheet can accordingly be much tidier.

This is all because there's no getting around the problem of specifying, for example, which elements other than the divs should be pulled in with them, which should be given their own page sequences, which should be grouped, etc.

It's really beginning to sound like a grouping problem. This too is tractable -- but will be much more so if your source is normalized into a tighter model. (Grouping problems need tight models!)
