Yes I should have said if there is no dtd. In theory if there is a DTd
that references a an external file that the parser does not read that
could have defined the entities (but doesn't) it is still well
formed. However you will be hard pressed to find a parser that accepts

<!DOCTYPE x SYSTEM "x-secretprotocol:rubbish">
<x>
&wellformedexample;
</x>
