You don't use a backslash to escape quotes (or other characters) in XML/XPath. The line above should result in an XML parsing error. For replacing a single quote, try <xsl:value-of select="translate(../@OrganizationCode,'"','B'"/>
Look up "predefined entities" and "character references" in your XML book (you have read one, haven't you?).
