Did you even try to search for "doctype" in <http://www.w3.org/TR/xslt>?

See <http://www.w3.org/TR/xslt#output>: set the output method to "xml", create the elements into the result tree in XHTML namespace, and set the doctype-public and doctype-system to their corresponding values. Also, add "ms" prefix to the exclude-result-prefixes attribute of the xsl:stylesheet element <http://www.w3.org/TR/xslt#stylesheet-element>, the XHTML DTD doesn't allow that namespace declaration.
