It's a general feature of xslt that templates just say what to do _if_ template matching is applied to a matching node, just defining a template doesn't on its own cause anything to happen, you need to apply templates to the node (or at least allow the default template for teh parent node to implictly apply templates to this node). If you want to apply templates to the children of xforms:instance replace <xsl:copy-of select="child::node()"/> by <xsl:apply-templates mode="copy"/. You can also almost certainly just write the match attribute as match="xforms:instance" unless you have other instances of that element elsewhere in the document that you need to handle differently.
