One of my clients is a database shop. They are in the habit of handing me ERD diagrams (to show the relationship of the tables) and lists of field names inside the tables. I still end up doing a fair bit of interviewing to work out the output details, but the ERD diagrams do make a useful starting point for the conversation. From those, I can create the schemas.
With my other clients, they tend to tell me what they want to appear in their documents (my work is mostly document-centric), and then I ask them where I can find that info. You can tell that I evolved my business out of technical writing, as interviewing folks to get the information I need is second nature to me. It's hard to do anything without a good set of requirements (the input and output that other folks in this thread have mentioned), so that's where I start.
I would be very interested in knowing what other folks are using, too - great topic!
