<xsl:template match="section/title">
<h1><xsl:if test="generate-id(.)=generate-id(key('page',@page-num)[1])"><xsl:value-of
select="generate-id(key('page',@page-num)[1])"/></xsl:if></h1>

assuming that you apply templates to title elements, that will make
empty h1 elements (which you never want, surely) except for the first
elememnt in the document to have each unique value of the page-num
attribute which will get the string value of the element. putting
<xsl:value-of select="generate-id(key('page',@page-num)[1])"/>< is just
a long/slow way of saying <xsl:value-of select="."/> since you ave
carefully tested that teh node returned by the key is in fact the same
node as returned by .

Without knowing what your input looks like or what your output should
look like, there's not a lot more help we can give. The above test does
correctly test for the first occurrence of eacg page.
