Saxon usually uses entity names for the html output method, are you sure
it is not in your case?

But that is different: the HTML 4 DTD does not define the apos entity
name (unlike XML) so if you write a ' as &apos; in html that is a syntax
error, so you would not expeect the XSLT system to do that. It only uses
names for characters which have a defined name in HTML.
