just change the second key to

<xsl:key name="ce" match="*[not(self::p|self::t)]/change-end" use="generate-id((preceding::*[self::p|self::t] except preceding::*[self::p|self::t]//*[self::p|self::t])[last()])"/>

so that nested elements are not counted and so you pick up the last element _end_.
