Just a thought.  How about using the "Select" switch as reverse, with it
active as long as the switch is held down.  This is more accessible than
the difficulty switch on newer units, plus it returns to off easily.  If
you playing barefoot, you can even press it with your big toe. :)

That only solves things for 1 player modes. It also deviates from classic 2600 UI where select is always active as game select.
