:) I wonder as well.
This was the first template I tried. What it does is, first it will print out all the <chapter_title> content and then content. But, I will give it another shot.
Thanks
Norm
<xsl:template match="chapter_title"> <fo:block font-size="14" font-weight="bold"> <xsl:value-of select="."/> <xsl:apply-templates select="sub"/> </fo:block> </xsl:template>

how long are trying to reach your aims? You seem to have a very big patience ;-)
Here are the templates (I assume, that your FO syntax is correct):
<xsl:template match="chapter_title"> <fo:block font-size="14" font-weight="bold"> <xsl:apply-templates/> </fo:block> </xsl:template>
<xsl:template match="sub"> <fo:inline vertical-align="sub"> <xsl:apply-templates/> </fo:inline> </xsl:template>
