DSSSL WWW Enhancements paper updated

I've updated by DSSSL WWW Enhancements paper
(http://www.jclark.com/dsssl/web-dsssl.htm) to include some more proposed
solutions (in particular for floating images and other flow objects into the
margin).

I have been disappointed that there has only been very general discussion of
syntax issues and no detailed discussion of any of the proposals I have
made.  Surely you don't all agree with everything I have proposed?
