I've been contemplating a 2-pass scenario: Jade does the first pass
generating special markers for missing Winhelp code. Then a Word
template macro does the second pass, changing the markers to the
correct format.
Maybe something like "{footnote # ...}" gets output by Jade, then
Word translates that to a # footnote. "{du ...}" ->
double-underlined text, etc.
