Modular DocBook Stylesheets and bibliographies

Hello. I'm having trouble getting bibliographies to show up in either my
html or print documents. I'm sure it's a usage problem, but I'm not
seeing what I'm doing wrong.

My starting point was to copy an example from the docbook element
documentation on the ora web site.

The "bibliography" section is included in the document at the same level
as "chapter".

The symptom is that the "bibliography" title, an introductory paragraph,
the "bibliodiv" titles, and the "bibliodiv" introductory paragraphs show
up, but the "bookbiblio" sections do not appear in the output. jade does
not generate errors on this. I've tried stripping out various pieces,
but have not stumbled upon the source of the problem.

I am running db104.zip, but have had the same problem in (at least) the
last several releases. I recall that at one time I got this to work, but
that may be my imagination :)

I've included a shrunken version of my bibliography sources. Any hints
would be appreciated. TIA
