Specifically, these inline-only page headers and footers are a characteristic of the simple-page-sequence flow object class. What we need to resolve this (and many other problems relating to pagination) is an implementation of the page-sequence flow object class. Page-sequence looks fearsomely generalised and powerful to me, and it may be that supporting the full page-sequence model is way beyond the abilities of current back-ends. Has anyone looked into this? If there is, from a technical point of view, a feasible way forward (perhaps a limited implementation of page-sequence), perhaps some organisation might be willing to follow ISOGEN's excellent example, by sponsoring the development of support for this flow object class (in return for the community's everlasting gratitude!)?
