By using (current-node-page-number-sosofo) in the toc mode during the
processing of the target node.  I usually combine this with a live link as
well along the lines of the following:

(mode toc
    ...
    (element (section title)
        (make paragraph
            (make link
                destination:    (current-node-address)
                (process-children))
            (make leader
                (literal "."))
            (make link
                destination:    (current-node-address)
                (current-node-page-number-sosofo))))
    ...
)
