Syntax to include graphics in DSSSL style sheet

I actually have two questions.

How do I insert a graphic into a DSSL style sheet - I have two that will sit
one above the other on a page with a comment below each,

and

How do I insert a graphic on the first page, with the text running over it -
ie it is a cover graphic with the title laid over the top.

I am using JADE with a self constructed DTD, and outputting to RTF.
