TOC with html32hc.dsl

I'm having troubles getting the Table of Contents generated by the HTML
3.2 style sheet.

I get a bunch of the following in my TOC in Word:

1. Error! Bookmark not defined.	 .......................  2

Anyone else have problems like this?
