Not a mistake, but possibly a bit confusing.

The list of elements that "this element may be contained in" does include
<sec-meta>.
Elements that contain <sec-meta> allow <kwd-group> inside the <sec-meta> but
not
outside the <sec-meta>.

Since <app>, <bio>, <boxed-text>, and <notes>, can contain <sec-meta>, in
these
the element <kwd-group> is allowed inside <sec-meta>, that is,
one level down from the top:

 bio
  sec-meta
    kwd-group
      kwd


So, <bio> and the rest of your list do contain <kwd-group>, just not at the
top level.
