Hi Ken,

I don't think that you will find any definition of "proper" 
superscripts. If you run out of asterisks, swords and daggers, you can 
always resort to superscript numbers or letters, as these are widely 
used across both printed manuscripts and online systems.

As for the rendering of empty cross references that point to items 
without labels, the rendering system can choose to ignore them or come 
up with meaningful visual cues based on publisher preferences. I doubt 
that there are any guidelines or any sort of standardization around that.
