I use the following as my document element start-tag:
<ixsl:stylesheet
xmlns:ixsl="http://www.w3.org/TR/WD-xsl"
xmlns:xsl="quote:http://www.w3.org/TR/WD-xsl"
result-ns="xsl">
Works like a charm with XT :-)
