Try this as the template for the Client element:

   <xslt:template match="Client">
     <FORM>
     <INPUT VALUE="{./NAME}">
     <INPUT/>
     </FORM>
   </xslt:template>

(Caveat: Haven't tested this -- away from my "XSL workplace" at the moment.)
