The logic for handling precedence _ought_ be the same as in a ContentML to Presentation ML stylesheet didn't it? that is if you took one of those, eg http://www.inrialpes.fr/opera/people/Emmanuel.Pietriga/mathmlc2p.html and just removed <mrow> <mo> and friends, wouldn't you be most of the way to what you need?
