your stylesheet has to be well formed XML.
<INPUT maxLength=7 name=test size=14>
is not well formed: attribute values have to be inside " " or ' '

Also you don't need xsl:attribute, see the thread ealier this week.

You want something like:

<INPUT maxLength="7" name="test" size="14" value="{TITLES}"/>
