If I have a multi-column layout, is there a way to have an image or

Yes, but in the simple page geometry available in XSL-FO 1.0 you only have the choice of spanning all columns, not just some of the columns. The act of spanning all columns splits the main reference area into separate span reference areas within which columns are balanced.

If so what processor supports this?

Both Antenna House and RenderX support the span="all" property.

I hope this helps.
