This is a null pointer exception. You can add -d to get a full stack trace, just in case. In case of errors, it is recommended to run the XML+XSL through Xalan or another XSLT processor standalone. This way you get a proper error message if the problem is with the XSL.

For example, this is a syntax error (xsl:text must not contain any markup). Just remove the xsl:text element.
There are a few unplesant problems which due to bugs in the Xalan release distributed with FOP cannot be found. A run with Saxon 6.5.4 or the most recent Xalan will find these. In your case, it looks like you have more problems, perhaps of the unpleasant kind, in your XSLT, because the problem mentioned above does not cause a NPE.
