Are you sure that this template is being applied at all, and that it isn't some other template generating your output, due to namespace differences or specific apply templates or priorities elesewhere in the document that you haven't shown us. Try that document with a complete stylesheet on a couple of different xslt systems, if they give different results it may be a processor bug, if they give the same result it's probably a bug in your stylesheet, but youll have to provide more information on what the stylesheet looks like.
