So I'm expecting with-param to do something it's not designed to do?
Calculate a new value for each node?

  <xsl:apply-templates select="section | comment | instructions">
    <xsl:with-param name="section-number" select="???" />
  </xsl:apply-templates>

The expression in ??? is not designed to give a different value
depending on the current node?
