Just a guess, but perhaps the document in which the <fo:inline appears is used to generate another document which is then parsed to produce the final output. The first parse would replace the first &amp;amp; with &amp; and the second would replace the resulting &amp; with &.

tried
<fo:inline >Report: Profit &amp; Loss</fo:inline>

and

<fo:inline >Report: Profit & Loss</fo:inline>

with no luck.
