XSLT goes beyond XML Namespace requirements by needing the use of prefixes

I do not see a declaration for xmlns:str above.

You probably also want exclude-result-prefixes="str" as well in the above.

You are using the prefix in the above attribute, so it must be in scope 
(according to XSLT rules, not according to XML rules).

You probably want exclude-result-prefixes="str" here again as well.
