us, so it's impossible to tell exactly why it's happening. How are these

condition nodes selected for processing?

But why you are matching only first condition nodes, and then doing
their 
following siblings with exactly the same code? Wouldn't it be easier to
say 
simply

<xsl:template match="condition">
   <eqpconds>
     <eqpconds-setup-item>
       <condition>
          <xsl:value-of select="."/>
       </condition>
       <itemref>
          <xsl:copy-of select="link"/>
       </itemref>
     </eqpconds-setup-item>
   </eqpconds>
</xsl:template>

and then make sure that all the condition nodes you want to process are 
selected properly (something that might be easier now)? Maybe there's a 
reason ... in any case there's more to this problem than what you've
shown us.
