  Something like

<xsl:for-each select="//*[action='change']">
  <xsl:if test="add and delete">
     <xsl:if test="add/c = delete/c">
        ...
     </xsl:if>
      <xsl:if test="add/d = delete/d">
        ...
      </xsl:if>
  </xsl:if>
</xsl:for-each>

(this is not tested)
