In case anyone is working on a solution to the problem presented in my
last post, I thought it only proper to inform you that I have discovered
an answer.  It follows:

I inherited these files recently.  From what I have learned, the xslt is
processing as it should.  I have not, however, been able to figure out
how to get the desired output.
