Oy! Thanks so much -- that was exactly what I was over looking.  I've
switched the xsl to:

<xsl:template match="table">
 <xsl:for-each select = "./tgroup">
  <fo:table table-layout="fixed" width="100%">
   <xsl:for-each select="./colspec">
    <fo:table-column column-width="{@colwidth}in"/>
   </xsl:for-each>
   <fo:table-body>
    <xsl:for-each select="./tbody/row">
     <fo:table-row>
      <xsl:for-each select="./entry">
       <fo:table-cell>
        <fo:block line-height="16pt" text-decoration="underline"
font-size = "10pt">
         <xsl:value-of select="." />
        </fo:block>
       </fo:table-cell>
      </xsl:for-each>
     </fo:table-row>
    </xsl:for-each>
   </fo:table-body>
  </fo:table>
 </xsl:for-each>
</xsl:template>

...and it works like a charm!
