We just did a similar application. Formatted in a two column table on a single page that was 17in x 11in with appropriate margins in the table cells. Formatted that to the area tree (XEP Intermediate Format) and then some simple code to manipulate XML was applied that file, changing it to a two page document by a mere examination of the "x" location in the area tree for each component rendered inserting "right hand" pages and putting the "right hand" content into those pages. The header/footer for each page was put into original and copied to each page in the conversion process. If you would like some assistance in setting up something similar, just contact me off list.
