First, many thanks to all those who responded to my query regarding
CDROMs
and copyright.  If there is an interest, I will be happy to post a
summary
to the list.



I have been presented with another copyright question from our faculty:
does
anyone have sample letters of permission or templates for letters of
permission?  I found one from the University of Texas System, but am
looking
for other successful samples.  Our faculty is also wanted to see if
anyone
had copies of letters they have received from publishers granting
permissions.
