(1) The example in 2.7.8 counts "chapter|section|subsection", yet the
format is only "1.1." ... should the format rather be "1.1.1."? If so,
would the incorrect version used in the example generate an error
(unmatched level counters), or would the subsection titles all be numbered
with the section numbers (thus losing the distinction of subsection in the
rendered result)?
(2) Later in the examples in 2.7.8, would the given snippet:
